Our 24-hour professional staff provides exceptional, individualized services to each of our residents.

• Ongoing communication between staff and residents’ family, power of attorney for health care, physician, legal representative and case manager
• Health monitoring by nursing staff, 24-hour on-call staff and consultation with physician medical director
• Initial health assessment to determine an individual service plan
• Changes in resident functioning, nutrition or cognitive status are recorded and monitored by trained staff, including nursing staff, and reported to each resident’s primary care physician
• Weight and blood pressure are taken each month
• Residents with specific disease conditions are monitored more frequently and as directed by resident’s physician
• Assistance with medications
• Assistance with personal care, including bathing, dressing, grooming, incontinence, hygiene, and ambulation
• Routine wash, set and styling of hair is provided on a weekly basis, or as needed/requested by the resident or resident’s family
• Assistance with laundry, housekeeping, shopping and making phone calls
• Coordination with hospice and home health agencies, if needed
